Sick certificates

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.   See link below to download form.

If you have used an SC2 self certificate and are sick longer than seven days you will need to make an appointment to see the Doctor before getting a further sick line.

If you are currently getting sick lines from the Doctor please complete the following form.  Please be aware sick lines can not be pre dated it can only be done from the day it is needed.  Please allow three working days for you sick line to be ready.
